The opening credits for the second season of The Quintessential Quintuplets makes it clear they will focus on Fuutarou realizing he's met one of these sisters before and fell in love with her at first sight.
Last season ended with Fuutarou getting a bad fever, so obviously he woke up in the hospital with vague memories of the Nakano sisters visiting him, feeling like he met the head doctor at the hospital before, having dreams of memories of when he saw that man with a pretty girl his age, and all the sisters bothering him as they chased down Nino and Itsuki to take their vaccination shots. Miku is still best girl, but Ichika is circling her territory.
Fuutarou came by the Nakano sisters' apartment after he got better and received double shock from seeing one of them in a bath towel and that girl hitting him with a stack of failed quizzes. Fuutarou needed to figure out which sister it was so he could admonish her for doing so poorly on her tests. Obviously, Fuutarou couldn't tell them apart. Miku even offered to show him where her mole is to test his recognition, but the other sisters nixed the idea because Fuutarou hadn't even seen any moles with that bath towel encounter. Fuutarou solved the mystery of the girl in bath towel as Ichika, but he realized the five tests belonged to all the sisters. They were all in trouble for bad grades! Miku was still mad that Fuutarou called her by the wrong girl. Don't make that poor girl pout, Fuutarou!
The end credits confuse us by making us think each of the Nakano sisters will wear a white dress like the young pretty girl Fuutarou met during his trip to Kyoto.
