Way to troll our heartstrings. A mackerel bone caused Grandma's coughing and bleeding, not some wasting disease.They mined a lot of material out of the cure for Keiichiro's curse of breaking out when he lusts after a woman. A virgin kiss is all it takes, but Kiriko says that if Keiichiro got into that situation, he'd probably lose his virginity at the same time. Grandma had the best line: "It's been so long, it's probably grown back already!" Of course, a maiden's kiss has to be about feelings, so a desperate Keiichiro got what he deserved from Kiriko. Well, the boy learns how to treat women, eventually.
I think it's pretty clear Hyakusuke is going to end up with Chiharu. He's so mean, making her have feelings for him because he defended her honor, or something.
The Lantern Festival was a good device for Grandma to tell Kiriko about his backstory and his mother, which also reinforced her mistaken idea that Grandma was going to die soon and had to tell her all this stuff. Again, all that coughing up blood business was well done.
And at the end, when Keiichiro finds his curse gone, he wonders how it happened. If Chiharu didn't do it, did Grandma really steal a kiss?
No way! But if Chiharu and Grandma didn't kiss him, could it have been...? The boy is still dumb, but I'm sure he acknowledges that Kiriko kissed him, but it will be an unspoken thing between them. Soft feelings can build on that.

We've done a lot of ogling Kiriko, but with the sad chance of Grandma dying too soon, and knowing the circumstances of a Shogun in love with a concubine's daughter, but not able to acknowledge her, and then Keiichiro being raised by Otomi after his mother died soon after his birth, any maiden's heart would soften.
