Ruka accused Chizuru of being a rental girlfriend and when Kazuya tried to convince her to keep it a secret, "things happened," and it turned out Ruka was also a rental girlfriend.
For some reason, Ruka looked for Kazuya on campus and wanted to be his girlfriend, even letting Kazuya's other sad sack friend's secret out about their rental relationship. Then she followed Kazuya home and found him talking to Chizuru. All sorts of things going wrong for how rental girlfriends are supposed to meet with their clients.
Chizuru encouraged Kazuya to go out with Ruka since she was so adamant about it. In one of the few things this show did right for adapting a manga, Ruka's flashback explanation for her heart condition, and how Kazuya is the first person or thing to make her heart go pitter-patter, appeared as a montage over the closing credits. Now we're all caught up, but Kazuya doesn't know any of this yet. That's fine. That sad sack panicky liar doesn't know anything.
